Clinical Lead Cardiac Service

Leeds Community Healthcare NHS Trust

  • Community Cardiac Nurse Specialist in Leeds for 11 years, and Clinical Lead for the Cardiac Service for the past 4 years. 

  • The Community Cardiac Nurses in Leeds are ‘dual role’ managing both Heart Failure and Cardiac Rehabilitation. There are 25 team members including a Physiotherapist, Mental Health Practitioner, and Healthcare Support Workers, alongside the Nurses.

  • The team see patients either at home or in one of the 25 clinic settings around the city. They also provide a Cardiac Rehabilitation exercise and education programme in 5 Leisure Centres which is offered to all patients. 

  • After securing funding from the British Heart Foundation for a 2 year pilot project, we set up a Community Intravenous Diuretic service in 2011 which continues to be an established part of the Cardiac Service.

  • The Community Service has close links with the acute trust and runs monthly MDT meetings with the 2 Heart Failure Cardiologists, and a Consultant in Palliative Medicine from the local Hospice. They also run weekly ‘virtual clinics’ with the Cardiologists.
